What patient outreach is and why the list falls apart

Outreach is the front desk word for one concrete thing: take the list of patients with something pending and call them again. The patient has already been seen and already has a written order, so the work starts where the consultation ended. US preventive care splits the same job in two, reminder and recall: the first tells a patient that care is due, the second that it is late, and the Community Preventive Services Task Force recommends both.

Who builds the outreach list?

  • Someone with database access runs a query and drops it into a spreadsheet.
  • The spreadsheet is split among the schedulers who have gaps between inbound calls.
  • Each of them notes the outcome her own way, if she gets to note it at all.
  • Nobody goes back to the ones who did not answer, because Monday brings a fresh spreadsheet.

The list ages on the way. By the time the call goes out, some of those patients have been seen elsewhere, some already have the appointment booked, and some have changed phone number. Inconsistent call disposition makes the three cases impossible to separate, so the team ends up not knowing how much of its effort landed.

Adding phones raises call capacity and leaves untouched the reason the list gets rebuilt every week. That is why a call center usually ends up administering the same backlog with more people inside it.
