A lipid panel measures cholesterol and triglycerides, and the fasting rule you will be given changed in the last decade

Few orders generate as many calls to the front desk as this one, and nearly all of them ask about the instruction that comes first, which stopped being the same everywhere.

What does a lipid panel group together?

Four measurements from one blood sample: total cholesterol, HDL cholesterol, LDL cholesterol and triglycerides. Some catalogues add VLDL cholesterol or an index calculated from the other values.

Total cholesterol also sits inside the metabolic panel. Seeing the same name in two services on one sheet convinces many people that one of them is enough, and the other is lost.

Do you have to fast or not?

The European Atherosclerosis Society and the European Federation of Clinical Chemistry and Laboratory Medicine published a 2016 joint consensus statement recommending non-fasting blood samples for routine assessment of plasma lipid profiles, and describing situations where fasting sampling remains useful, among them very high triglycerides in the non-fasting sample.

That consensus was not adopted at the same pace by every laboratory. Today you will find centres that demand strict fasting, centres that demand it only when other tests share the sheet, and centres that do not ask for it. None of them is out of line: the centre's protocol governs.

What does it get confused with on the order?

  • Plain «cholesterol», which in several catalogues is a single determination rather than the full panel.
  • «Extended lipid panel», which adds measurements and is a different catalogue row.
  • Haemoglobin A1c, which usually accompanies it in metabolic follow-up and has its own preparation rules.
  • Liver tests, because they share the word «panel» and are booked in the same unit.

Why does this order go unbooked more than others?

Because it is almost always given to someone who feels fine. No symptom pushes, no pain reminds. The order lands on the bedside table and competes with the rest of life.

Add the failed trip. Anyone who arrives without the right preparation is turned away, and the odds of them coming back unprompted drop sharply. Every wasted trip turns a live order into a care gap.

What should you do with the order you have?

  1. Confirm the name.

    Read the centre the full line exactly as written, abbreviations included.

  2. Ask for the preparation in writing.

    By message, email or on the booking receipt. If other tests share the sheet, ask for the combined instruction.

  3. Book the follow-up.

    A lipid panel exists so that somebody reads it with you. Without that appointment the test is half done.
